Stable housing is the key to stopping the spread of HIV, providing the platform for receiving healthcare, stabilizing mental health, treating addictions, and re-connecting with families and social networks.
People moving into Clare Housing work with supportive services staff to develop an individual housing plan based on their background, needs and personal strengths. Our staff then provides ongoing support for residents in daily living skills, managing their health and medications, developing positive community-based support networks and accessing additional resources and services.

Supportive Housing Apartment BuildingsClare Housing owns and operates four supportive housing apartment communities in Minneapolis, providing supportive housing to more than 150 residents annually. All buildings are staffed 24/7, helping keep the apartment community safe and secure.
About 20% of our apartment residents need a higher level of medical care and are enrolled in our Home Care program, receiving onsite supportive services, such as nursing care, medication administration, hands-on assistance with activities of daily living, and help with building independent living skills.

Scattered Site HousingWe partner with landlords to provide 75-85 rental units within market-rate and nonprofit housing developments throughout the 7-county Minneapolis-St. Paul metro area. Our staff supports residents in maintaining permanent housing and coping with life challenges. Many of our Scattered Site residents are families living with children.

Community-Based Care HomesOur longest running program, these are three homes that are staffed 24/7, each serving four residents in need of reliable care and support to live outside of a nursing home. Homes are located on residential streets in south Minneapolis. Each house is cozy and unique while providing a fully accessible environment for residents. Staff provide rehabilitation support to those needing to stabilize their health before returning to independent living, long-term care to those disabled by HIV and other conditions, and end-of-life care when needed.

Indigo HouseIndigo House offers immediate housing to people who are part of the homelessness encampment cluster, offering five private units for 1-2 people each. Within a shared home with 24/7 service staffing, Clare Housing case managers focus on building relationships and helping residents meet their basic needs. After establishing a foundation of safety, rest, and trust, staff and residents work together to explore future housing goals and options.

Hotels to Housing
Hotels to Housing helps people living with HIV and experiencing unsheltered homelessness move off the streets quickly. Clients move temporarily into hotels followed by a move into permanent supportive housing within weeks. No lengthy paperwork or screening process is required, so clients are able to have their most pressing needs met immediately as they transition into housing.

Greater Minnesota Housing Assistance
Outside of the Twin Cities metro area, this program provides people who are living with HIV/AIDS emergency short term assistance for up to three months. Funds are available to eligible people and families as emergency assistance for short-term rent, mortgage, and utility payments (STRMU). STRMU serves people in Greater Minnesota where there are very few HIV specific resources.
How to become a resident?
Clare Housing maintains several specific waiting lists for our three supportive housing programs. Submitting a formal application for each program is required, and we recommend that prospective residents work with their HIV case manager or social worker for assistance in filling out these applications.
